Upon submitting your vinyl order, please send this completed form and any relevant licensing documents or label agreements to hello@cherubrevolution.com. Cherub Revolution cannot ship test pressings until the order is copyright cleared.
Are you the copyright owner of the master recordings? If no, you must provide licensing.
Mechanical licenses are required when the album contains a cover song or material licensed from the copyright owner.
Mechanical licenses for cover songs can often be acquired through the Harry Fox Agency. If not available through Harry Fox, please contact the publisher to obtain the license(s).
Sampled recordings require a master license of the original recordings in addition to a mechanical license. These documents are required regardless of sample length.
Are you the songwriter of 100% of each composition? If no, you must provide mechanical licenses or a waiver signed by ALL songwriters of ALL compositions.
Are you the copyright owner on all of the artwork? If no, you must provide licensing from the owner.
By signing this form you are making the representation and warranty you have all the rights to manufacture the records, reproduce the artwork, master recordings and compositions embodied thereon and will indemnify Cherub Revolution for any third-party claims inconsistent with that representation and warranty.

Submit this digitally signed form along with any licensing documents to hello@cherubrevolution.com with subject line: "Copyright Compliance — [Your Project Title]". You may also bring the completed form in-person to Cherub Revolution in Tulsa, OK.
Why This Matters
Under the Audio Home Recording Act and US copyright law, pressing plants are required to verify that all content on a pressed record is properly licensed before manufacturing.
Pressing copyrighted material without authorization exposes both the artist and the pressing plant to significant legal liability.
Cover songs, sampled recordings, and licensed artwork each require separate documentation, even if only a portion is used.
